The CARE Coalition is a movement to unite and engage healthcare professionals to more effectively and efficiently provide quality patient care, improve outcomes, and cultivate collaborative processes among the many different channels of healthcare delivery. A portion of the proceeds from the CARE Coalition NEO Conference 2019 will go to benefit the Direction Home Akron Canton Foundation.
About the Foundation The Direction Home Akron Canton Foundation was created with the mission of enabling older adults not eligible for government assistance to remain independent and secure at home through access to a variety of supportive services. The Foundation was developed in 1999 to help these older adults achieve their long-term care goals through the Care Coordination program.
